Services
After initial contact, one of our Registered Nurses will contact the client or family member to schedule an assessment. Based on the assessment, and with participation from the client and/or family member, the RN will develop a care plan that fits the needs of the client. The care plans are updated to reflect changes in the client's needs/condition. Every effort is made to assign a Service Provider who will be most compatible with the client.
